Job Functions Conduct pre-emplacement interviews with residents to understand their needs, work with them to set goals, develop case plans to address concerns Implement, review and modify case plans Prepare and maintain Individualised Service Plans (ISPs), progress and closure reports Provide casework and counselling services to residents and their families Refer residents and their families to appropriate agencies for further intervention Ensure that the halfway house's Programme meets the mission, objectives and procedures of case management services 2. Candidate Requirements At least a Degree or Post Graduate Diploma in Social Work or related disciplines from recognised universities Registered as an Accredited Social Worker or Social Service Practitioner will be an advantage At least 2-3 years of relevant casework and counselling experience in the field, preferably working with offenders Good interpersonal and communication skills (oral and written) Able to work independently and at the same time a team player Self-motivated and takes initiative to drive improvements and developments Proficient in Microsoft Office applications such as Word and Excel, as well as able to perform basic statistical analysis 3. Working Hours 5 days work week Monday to Friday - 9am to 6pm 4.
