JOB DESCRIPTION: SUMMARY OF POSITION Coordinate services are referred by Department of Child Services and provided by Centerstone across all programs. This included direct contact with DCS referral source, arranging access to services and oversight of the DCS case. ESSENTIAL DUTIES & RESPONSIBILITIES Facilitate meetings with Centerstone staff who provide services to DCS families, and act as an advocate when necessary. Maintain necessary communication with all internal service providers and other involved parties. Obtain DCS Case Plan and ensure all Centerstone services are in alignment with DCS. DCS receives ongoing communication regarding status of referrals and impact of services. Centerstone staff receives communication about the overall status of the DCS cases, and internal services are coordinated. KNOWLEDGE, SKILLS & ABILITIES Effectively communicate via written, verbal, in person and virtual methods.
