This role focuses on early identification, response, treatment and documentation related to the high-risk diagnoses sepsis and stroke, while also participating in quality initiatives, education, and process improvement ideation. This role affords the ability to intervene at the elbow to ensure excellent care is delivered to patients at risk of sepsis and stroke and support their best possible outcomes. Collaborate with physicians, nurses, and other healthcare professionals to support evidence-based care delivery and improve patient outcomes. Provides clinical oversight and direction to frontline staff in identifying and managing patients with the condition's sepsis and stroke. Participates in patient care activities for high-acuity cases, rapid response events such as sepsis alerts, regional stroke alerts and care coordinating. Monitors Emergency Department for signs of clinical deterioration and escalates concerns appropriately. Responds to clinical alerts and assists with timely and accurate documentation related to patient status and care provided. Educates staff on best practices, quality standards, and clinical protocols to enhance disease recognition, documentation, and management. Contributes to quality improvement initiatives by identifying performance gaps, supporting interventions, and tracking progress. Serves as a clinical resource across the enterprise for best practices related to disease-specific care and patient safety. Provides direct treatment support during emergent patient scenarios as needed to ensure optimal outcomes. Supports occurrence reporting and helps identify potential patient safety issues, participating in follow-up and resolution efforts. Maintains current knowledge of clinical and regulatory standards related to quality and safety. Consistently deliver the best in customer service and patient care. Monitor ED for patients who are developing symptoms of sepsis and escalate concerns. Determine responding order of operations by recognizing areas of greatest need. Recognize gaps in treatment and escalate missing orders and documentations to the care team. Provide real-time feedback to staff on bundle compliance, appropriate triage screening, OPA responses, orderset use, etc. Work is accomplished without considerable direction. Frequently, the QRN, must adapt and respond to organization needs, current responsibilities and changing priorities. What We Require: Education: Graduate of an accredited School of Nursing required. Bachelor's degree Preferred. Experience: Three years of RN experience required. At least 2 years of experience in relevant specialty or department, as defined by the department, desired. (ex: hospital setting, vascular, rehab, long term care, home health, cardiology, ambulatory, surgery) Licensure, certification, and/or registration: Current VA RN license required. Must obtain & maintain appropriate certifications as defined by the department. Life Support: AHA BLS-HCP required within 1 year of hire. Other Minimum Qualifications: Effective interpersonal, communication, and team-working skills required. Strong critical thinking, conflict resolution, organization and effective judgement abilities required. Must successfully complete orientation and competency validation requirements. Must be able to meet essential job functions. Demonstrates excellent problem solving, interpersonal, communication, priority setting and work competency skills. Promotes a respectful, inclusive culture of service excellence. Excellent problem-solving skills and ability to exercise independent judgment.
