About this Role The Case Manager is a key member of the interdisciplinary treatment team, responsible for coordinating and advocating for comprehensive services that address the biopsychosocial needs of clients in outpatient substance use and co-occurring mental health treatment. Utilizing a trauma-informed, person-centered approach, the Case Manager supports client recovery through care coordination, resource navigation, and ongoing support to address barriers to treatment engagement and retention. Key Responsibiliti es (include but are not limited to): Conduct comprehensive case management assessments upon admission and at regular intervals. Develop and implement individualized care plans in collaboration with the client and treatment team. Coordinate access to social, financial, medical, legal, housing, and vocational resources to address social determinants of health. Collaborates with Peer Support Specialists to work together in assisting patients in addressing their social determinants of health Monitor progress toward care plan goals and update plans as needed. Maintain ongoing communication with internal clinical team members and external service providers to ensure continuity of care. Facilitate warm hand-offs and follow-ups to support treatment retention. Provide psychoeducation and support on navigating systems such as Medicaid, legal aid, child welfare, etc. Advocate for clients' needs while promoting autonomy and empowerment. Maintain accurate, timely documentation in accordance with Medicaid, Medicare, and CARF standards (including contact logs, service plans, progress notes). Participate in interdisciplinary team meetings, treatment planning, and staff development trainings. Identify and address potential risks to client safety and report concerns according to protocol. Safely transport patients to and from appointments, facilities, and designated locations using a personal vehicle while maintaining professionalism and patient confidentiality.
