Responsibilities

  • Therapeutic Interventions - Implement individualized treatment plans including therapeutic exercise, gait training, balance activities, and functional mobility tasks under the supervision of the PT.

Requirements

  • Accredited PTA Program graduate with an active California PTA license.
  • Current CPR/BLS certification.
  • Experience working with geriatric populations or in long ‑ term care, home health, or PACE preferred.
  • Ability to work independently, travel locally, and adapt to changing participant needs.
  • Strong communication, documentation, and teamwork skills.
  • Schedule & Work Environment
  • Per diem / as ‑ needed typically 8am to 5pm
  • Services delivered across Habitat Health PACE Centers (Sacramento, South LA, San Leandro, West Covina) participant homes.
  • Collaborative, mission driven environment focused on supporting older adults to remain safely in their homes.
