POSITION SUMMARY: The Social Worker MSW/LCSW is a clinician responsible for social work services for patients in various units of the Medical Center as well as serves in a consultative role to staff on the units in the medical center whenever needed for patient care. The Social Worker MSW/LCSW works as part of the interdisciplinary team caring/assisting patients and families to cope with plans and decisions that need to be made before, during and after hospitalization. Position: LCSW/MSW ER/ICU Department: Care Management Schedule: Full Time ESSENTIAL RESPONSIBILITIES / DUTIES: Provide s individual and/or group counseling/therapy to patients and/ or their families. Supervise s and train s graduate level social work students and other students when indicated and approved by the Director of Social Work . Provide s diagnostic evaluations and psycho-social assessments. Provide s referrals and/or follow-up services. Collaborate with clinical interdisciplinary team in f acilitat ing continuing care plans. Provide crisis prevention when needed to patients in various units of the Medical Center. Provides counseling for patients, family and friends whenever indicated at time of diagnosis, regards adjustment to illness, the emotional impact of diagnosis, and cha n ges in lifestyle . Offers continuity by maintaining a supportive relationship to patient and family throughout course of illness. On occasions when it is indicated , the social worker will counsel and support patients and other family in bereavement. The social worker will provide case management for patients. This will include resource findings, concrete services as indicated and will often involve referrals to community agencies and assistance in obtaining financial aid. P rovides documentation for social work activity in patient's record on a regular and timely basis. C onsults with members of the medical center team in regard to the psycho-social needs of patients. Participates in staff in-service. The social worker leads or co-leads a support group for patients and/or family members when appropriate . P articipates where possible in community organizations relevant to the population. M ake s a psycho-social assessment of as many high-risk clinic patients as time allows. T he S ocial W orker MSW/LCSW will also see patients at time of crisis and will assist where possible in cases of need for concrete services. The social worker will see any patient wishing to place their baby for adoption, referring them to the appropriate agency . A ssess the discharge needs of all patients admitted to their respective department assignment. Performs other duties as assigned. JOB REQUIREMENTS REQUIRED EDUCATION AND EXPERIENCE : Master's Degree in Social Wor k and 0-2 years of experience working with patients and families who are in crisis ; or equivalent combination of education and experience . PREFERRED EDUCATION AND EXPERIENCE: 1-2 years of experience with patients with major trauma, infectious disease, and/or emotional upset. CERTIFICATIONS , L ICENSES, REGISTRATIONS REQUIRED: Current Licensed Clinical Social Work in the state of Massachusetts CERTIFICATIONS , LICENSES, REGISTRATIONS PREFERRED: LICSW preferred KNOWLEDGE, SKILLS & ABILITIES (KSAs): Ability to work with patients and families who are in crisis. Compensation Range: $60,500.00- $88,000.00 This range offers an estimate based on the minimum job qualifications. However, our approach to determining base pay is comprehensive, and a broad range of factors is considered when making an offer. This includes education, experience, skills, and certifications/licensures as they directly relate to position requirements; as well as business/organizational needs, internal equity, and market-competitiveness.
