Responsibilities

• Address and complete provider consults proficiently, provide drug information to healthcare professionals and education to patients.

Stormont Vail Health Clinical Pharmacist Specialist uphold American College of Clinical Pharmacist philosophy of pharmaceutical care; which is blending a caring orientation with specialized therapeutic knowledge, experience and judgment for the purpose of ensuring optimal patient outcomes. The Clinical Pharmacy Specialist also contributes to the generation of new knowledge that advances health and quality of life. SPECIFIC. Will provide services to patients from birth to death. Education Qualifications Bachelor's Degree Bachelor of Science in Pharmacy from an accredited school of pharmacy. Required or Doctorate PharmD from an accredited school of pharmacy. Required Experience Qualifications 3 years Relevant acute care pharmacy experience or has successfully completed a Post Graduate-Year 1 accredited pharmacy practice residency. Required Successful completion of a Post Graduate Year 2 accredited pharmacy practice residency program. Preferred Skills and Abilities Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public. Ability to apply advanced pharmaceutical mathematical concepts including but not limited to the following: pharmacokinetic calculations, Child Pugh score, Cockcroft- Gault calculations, and other pertinent calculations based on patient population. Ability to define problems collects data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of technical instructions in mathematical or diagram form and deal with several abstract and concrete variables. Spreadsheets, Internet/E-mail, PowerPoint, Microsoft Word and Database software Licenses and Certifications Pharmacist - KBP Must be licensed in Kansas within 90 days of hire. Required Basic Life Support - BLS Required upon hire Advanced Cardiovascular Life Support - ACLS Required upon hire Vaccine Certification is required.
