This is a great opportunity to support service redesign and deliver high‑quality, strengths‑based assessments across adult social care. What will your responsibilities be? Undertake functional assessments to promote independence and wellbeing Support transformation and improvement initiatives within adult services Complete Care Act-compliant assessments and reviews Recommend equipment, adaptations, and reablement solutions Work collaboratively with internal teams and partner agencies Contribute to efficient discharge planning and prevention of unnecessary care packages
