Undertake functional assessments to promote independence and wellbeing Support transformation and improvement initiatives within adult services Complete Care Act-compliant assessments and reviews Recommend equipment, adaptations, and reablement solutions Work collaboratively with internal teams and partner agencies Contribute to efficient discharge planning and prevention of unnecessary care packages
