We are looking for a Case Manager who can bring people together, navigate complexities with confidence, and advocate for the well-being of persons living with dementia. You may be a good fit if you: - Enjoy working closely with people from different backgrounds - Can balance compassion with professionalism - Communicate confidently with residents, families and healthcare professionals - Stay organised while managing multiple responsibilities - Are proactive and solution-oriented - Value teamwork and collaboration - Find purpose in supporting seniors and their families through important life transitions In this role, you will: - Coordinate care plans to ensure residents receive holistic and person-centred support - Partner with nurses, therapists, doctors and other professionals to deliver quality care - Build strong relationships with residents' families and serve as a key point of contact - Support admissions, care transitions and discharge planning - Monitor residents' progress and facilitate care reviews when required - Assist with audits, reporting and quality improvement initiatives related to social care services - Maintain accurate documentation and ensure compliance with relevant standards - Participate in multidisciplinary discussions and contribute to care planning - Advocate for residents' emotional, social and psychosocial well-being
