About the role

Position Details Location: Brick, New Jersey Schedule: Full-Time Starting Pay: $36.10 per hour Additional Shift Differentials Available: Earn an additional $2.00-$4.00 per hour based on applicable shift assignments Essential Responsibilities Provide nursing services and clinical oversight within residential group home settings. Assist with the development, implementation, and monitoring of individualized healthcare plans. Maintain accurate medical records, health assessments, medication administration records, and related documentation. Monitor individuals' overall health status and communicate medical concerns, changes in condition, or treatment updates to appropriate team members. Collaborate with residential leadership, healthcare providers, and interdisciplinary teams to support ongoing medical needs. Assist individuals during physician visits, hospitalizations, specialty appointments, and admissions to rehabilitation or nursing facilities when required. Support medication administration processes and ensure adherence to agency policies and healthcare regulations. Promote a safe and healthy living environment while advocating for the well-being of individuals receiving services. Participate in health-related training, education, and quality assurance initiatives as needed. Previous experience working with individuals with intellectual and developmental disabilities is preferred but not required. Why Join Our Team? Graduate of an accredited School of Nursing required. Current Licensed Practical Nurse (LPN) license issued by the New Jersey Board of Nursing required and must remain in good standing. Minimum of one (1) year of experience as a Licensed Practical Nurse required. Valid New Jersey driver's license required, with no more than five (5) points. Strong communication, documentation, and organizational skills.
