This is a career-defining role for a highly autonomous Palliative Care Physician or Palliative Care Nurse Practitioner who is passionate about delivering compassionate, patient-centered palliative care. You will be instrumental in optimizing the quality of life for our most vulnerable members, utilizing your expertise to manage complex symptoms, drive crucial advance care planning, and ensure dignity through seamless, timely transitions to hospice care. As a Palliative Care Provider, you will: Provide comprehensive palliative care to patients in their homes, focusing on pain and symptom management, improving quality of life, and offering emotional and spiritual support. Perform in-depth assessments of patients with serious or life-limiting conditions, develop personalized care plans, and manage their medical needs. Work closely with patients and their families to establish goals of care and ensure treatment aligns with their wishes, emphasizing patient autonomy and dignity. Work in collaboration with a multidisciplinary team (nurses, social workers, and other healthcare professionals) to ensure a holistic approach to patient care. Participate in a shared on-call rotation, as directed by the Senior Medical Director. Travel locally throughout Monmouth, Mercer, Middlesex, and Somerset Counties to provide in-home patient care visits. You should get in touch if: For Physicians: You have an MD or DO degree and are Board Certified or Board Eligible in Palliative Care, Hospice & Palliative Medicine, or a related field. You are located in or willing to travel throughout Monmouth, Mercer, Middlesex, and Somerset Counties, NJ. You hold (or are willing to obtain) an active New Jersey medical license, DEA registration, and CDS registration. You have a minimum of 3 years of clinical experience in palliative care, hospice, or a related field. Experience in home-based care, house calls, complex care management, or value-based care environments is strongly preferred. For Nurse Practitioners: You are based in or able to travel throughout Monmouth, Mercer, Middlesex, and Somerset Counties, NJ. You possess a BSN from an accredited program and a Master's degree in advanced clinical practice nursing or a post-Master's certificate. You hold an active New Jersey Nurse Practitioner license, active DEA registration, and active CDS registration. You have prior experience working as part of an inpatient palliative care team, hospice organization, home health organization, or complex care program. Experience in house calls, longitudinal care management, chronic disease management, hospice, or palliative care is strongly preferred. Board certification in Palliative Care or Hospice Nursing is preferred.
