Liquid Personnel is currently recruiting an experienced Locum Adult Social Worker to join its client's RIACT Department in Darlington. This dynamic role focuses on rapid response assessments and crisis intervention, aimed at supporting adults experiencing sudden health or social care needs. What will your responsibilities be? Rapid Response Assessments: Conduct urgent assessments of adults experiencing a sudden decline in health or social functioning. Determine eligibility for care and support under the Care Act 2014. Identify and address immediate needs. Crisis Intervention and Hospital Avoidance: Prevent unnecessary hospital admissions by arranging short-term care packages, reablement services, or community support. Support timely hospital discharges by coordinating care plans for safe return home. Safeguarding and Multi-Agency Coordination: Identify and respond to safeguarding concerns, including referrals and strategy meetings. Collaborate with health professionals, occupational therapists, and care providers to deliver integrated care.
