Requirements

WHAT YOU WILL LOVE DOING:Observes and monitors patient conditions and performs comprehensive assessments to develop an individualized care plan and adjusts as needs change.Evaluates the treatment and medical condition of the patient according to the plan of care.Determines if the level of care being provided meets the patients' needs and facilitates level of care changes including the management of after-hours Routine, General Inpatient, Respite and Continuous Care.Administers medication as prescribed by the physician and collaborates on required updates to the plan of care to achieve comfort and symptom control.Prioritizes visits for high-risk patients.Manages multi-disciplinary care as applicable while promoting continuity of care with collaborative determinations on level of care provisions, additional resource need identification, and schedule management.Educates the patient and family on the disease process using teach back methods to ensure patient and family understanding.Completes all clinical documentation following agency protocol and Medicare/Federal guidelines.Completes chart reviews and answer patient calls, to support patient care and regulatory compliance.HARD & SOFT SKILLS:Compassionate communicator with a positive attitude.Patience is a virtue when working with patients, families, physicians, and coworkers.Attention to detail is critical, as is being observant and following directions.Health insuranceDental insuranceVision insurance401(k)Paid time off
