Job Description: Registered Nurse (RN) - Casual Position Title Registered Nurse (RN) - Casual Reports To Director of Care (DOC) / Assistant Director of Care (ADOC) Job Summary The Registered Nurse (RN) is responsible for providing professional nursing care to residents in accordance with the standards established by the College of Nurses of Ontario (CNO), the Fixing Long-Term Care Act, organizational policies, and evidence-based best practices. Acting as the Nurse in Charge during assigned shifts, the RN provides clinical leadership, supervises Registered Practical Nurses (RPNs) and Personal Support Workers (PSWs), coordinates resident care services, and ensures the safe and effective operation of the home. The RN is responsible for resident assessments, medication administration, clinical documentation in PointClickCare (PCC), risk management, incident follow-up, infection prevention and control, and emergency response activities.
Responsibilities
- Clinical Care & Resident Assessment
- Perform comprehensive nursing assessments and ongoing evaluation of residents.
- Monitor and respond to changes in resident condition.
- Develop, implement, and evaluate individualized plans of care.
- Collaborate with physicians, nurse practitioners, pharmacists, and interdisciplinary team members.
- Ensure timely follow-up of abnormal findings and clinical concerns.
- Participate in admissions, transfers, discharges, and care conferences.
- Provide palliative and end-of-life care as appropriate.
- Medication Administration
- Administer medications and treatments safely and accurately.
- Monitor residents for therapeutic and adverse medication effects.
- Ensure compliance with medication administration policies and professional standards.
- Investigate and report medication incidents and near misses.
- Complete narcotic and controlled substance counts and follow up on discrepancies.
- Liaise with pharmacy services regarding medication-related concerns.
- PointClickCare (PCC) Documentation
- Maintain accurate, complete, and timely documentation within PointClickCare (PCC).
- Complete nursing notes, assessments, care plans, and clinical monitoring records.
- Utilize PCC modules and clinical pathways effectively.
- Monitor outstanding assessments and documentation requirements.
- Ensure documentation meets regulatory, legal, and organizational requirements.
- Risk Management & Incident Follow-Up
- Identify and mitigate risks affecting resident safety.
- Complete and follow up on incidents including: Falls
- Head injuries
- Skin tears and wounds
- Medication incidents
- Resident-to-resident altercations
- Responsive behaviours
- Missing resident/elopement concerns
- Infection prevention and control issues
- Utilize PCC Risk Management tools and monitoring pathways.
- Participate in investigations and corrective action planning.
- Report incidents according to organizational and legislative requirements.
- Leadership & Staff Supervision
- Act as the Nurse in Charge during assigned shifts.
- Provide leadership and clinical guidance to RPNs and PSWs.
- Assign and coordinate resident care assignments.
- Support staff in clinical decision-making and problem-solving.
