Pine Park Health is hiring Physician Assistants (Full-Time) to revolutionize primary care for seniors in the East Bay area. With dedicated care coordinators handling administrative work and RNs managing triage, you can focus on what you do best: comprehensive clinical care. Our collaborative team structure provides robust support for chart prep, patient education, and follow-up management, creating a workflow that enhances both provider satisfaction and patient outcomes. Responsibilities

  • Provide comprehensive onsite primary care to seniors in assisted living and senior living communities
  • Develop and execute personalized, patient-centered care plans that address the whole person
  • Partner with our Medical Director to refine clinical policies, protocols, and guidelines as our practice evolves
  • Deliver monthly Chronic Care Management (CCM) visits and oversee CCM documentation
  • Collaborate with senior living leadership and care teams to optimize care plan execution and patient outcomes

Requirements

  • Clinical credentials: PA certification with a genuine passion for geriatric medicine (prior geriatric experience preferred but not required)
