Location: Steam Plant - HQ Department: Administration Weekly Hours: 40 Status: Full time Shift: Days (United States of America) Job Summary: As a Quality Care Coordinator, you will play a key role in advancing value-based care initiatives by driving improvements in quality, closing care gaps, and enhancing patient health outcomes. You will serve as a liaison, advocate, and informational resource for patients, families, clinicians, and Care Teams to ensure care is coordinated, efficient, and aligned with quality goals. You will conduct outreach to targeted populations to promote preventive care and disease management. In addition, you will coordinate follow-up care, identify barriers that impact access or engagement, and connect patients to appropriate community, social, and health resources. You will obtain, analyze, and apply clinical quality and population health data to identify care gaps and close opportunities. This includes generating reports, conducting detailed chart reviews, abstracting relevant information, and entering medical records and clinical data into the electronic health record.
