The Exercise Therapist will develop exercise treatment plans in accordance to the patient cardiovascular and overall physical abilities and provide support for self-management cardiovascular risk factor management. Accountabilities: Function as a case manager for a designated group or program participants and as a liaison between the participant and their physician and other health care providers Develop exercise prescriptions for participant with cardiovascular disease Review exercise assessment results in developing exercise prescription and care plans Conduct exercise and educational classes Provide ongoing motivational support for self-management strategies with respect to long-term management of cardiovascular risk factors Apply principles of program planning, design adaptation and prescription in physical activity Counsel patients/clients regarding healthy behaviours, self-management lifestyle techniques University degree in kinesiology/physical education with successful completion of advanced cardiac physiology, cardiac rehabilitation and exercise physiology electives Member in good standing with their professional college or regulatory body Minimum of two years of relevant experience in cardiac rehabilitation Current ACSM Clinical Exercise Physiologist (CEP) and CSEP CEP certified Current BCLS certification with AED Current membership with Cardiac Rehabilitation Network of Ontario and Canadian Association of Cardiovascular Prevention and Rehabilitation Working knowledge of cardiovascular exercise physiology, ECG interpretation, cardiovascular medications, principles of behaviour modification and cardiovascular exercise therapy Good interpersonal skills with the ability to work as part of an inter-professional care team Experience in adult education and presentation skills Excellent oral and written communication Ability to work independently with minimal supervision Ability to travel between all Osler sites Must demonstrate Osler's Values of Respect, Excellence, Service, Compassion, Innovation and Collaboration Hours: Currently Days/Evenings, 8 hours, including weekends and statutory holidays (subject to change in accordance with operational requirements) SALARY: Minimum: $41.76 per hour Maximum: $53.50 per hour Internal application deadline: [June 11, 2026] #LI-SC1 #CS Osler values inclusivity and diversity in the workplace.
