Liquid Personnel is currently recruiting for a Social Worker to join a Mental Health Hospital Discharge Team based in Stockport.
What will your responsibilities be?
In this role, you will join an all-age adult mental health hospital discharge team, collaborating with Pennine Care colleagues. You will support timely and safe hospital discharges for adults with functional or organic mental health diagnoses. Your work will involve close collaboration with individuals, carers, health professionals, advocates, and third-sector organizations. You will conduct Care Act assessments, develop support plans, perform risk assessments, and carry out Mental Capacity Assessments and Best Interests Meetings. Additionally, you will prepare Social Circumstances Reports and attend Mental Health Tribunals to ensure comprehensive support and advocacy for those in your care.
