Job Description: The Nurse Practitioner, Ostomy and Wound Program reports directly to the Assistant Director, Wound Program and under the supervision of the program Medical Director. S/he is responsible for providing wound care and ostomy management and education to inpatients and outpatients. The Nurse Practitioner, Wound Program possesses a broad scope of knowledge and skills in ostomy and wound care to provide patient care and facilitate patient self-care management. Communicates with and collaborates with physicians, other hospital nursing staff, and other healthcare team members to provide consistent quality care for ostomy and wound program patients. The ostomy and wound program Nurse Practitioner practices as a member of the interdisciplinary team. The Nurse Practitioner works with a high degree of autonomy and is responsible and accountable for assigned patients as a case manager to plan, evaluate, and implement the care for patients within the framework of the State Nursing Practice Act. The Nurse Practitioner complies with all hospital policies and procedures while performing job duties and responsibilities including, but not limited to: participation in the program's daily operations; direct patient care; and providing education to patients and families. Educates, coaches and assists nursing team members to ensure optimal wound and ostomy care and efficient use of resources and documentation. Integrates knowledge of age and developmental specific care, institutional goals and standards, current best practice standards. Uses standardized procedures to independently examine, diagnose, and treat patients in collaboration with supervising physicians
