Responsibilities

  • Be embedded within the MCI program to focus on identifying, opening, and supporting families who are receiving CBHC MCI or intake services.
  • Work closely with the CSA team, family, and other collaterals in developing measurable family partner goals to support the family's Individual Care Plan (ICP) and related goals.
  • Engage caregiver(s) in activities in the home and in the community that are designed to address one or more goals/tasks on the family's ICP.
  • Assist caregiver(s) in meeting the needs of the youth through educating, supporting, coaching, modeling, advocating and guiding.
  • Teach caregiver(s) how to network/link to community resources and treatment providers.
  • Support caregiver(s) in advocating for services and resources to meet youth and family needs.
  • Guide, support and foster empowerment including, linkages to individual, peer/parent support and self-help groups.

Requirements

  • Associate's degree in human services or related field
  • Must have a valid driver's license and reliable transportation
  • Must be a parent, guardian or caregiver of a youth with behavioral health needs
  • Experience in navigating child/family service systems
  • Schedule: Monday-Friday, 10am-6pm (Flexibility is required to work earlier or later as needed to accommodate client needs.)
  • Eligible for 6% bilingual pay increase if fluent in needed language capacity.
