Performs delegated or other specialized functions as educationally prepared.
Escorts patients to and from various destinations.
Assists in maintaining and providing a clean, safe environment.
Orders, receives and stores supplies and performs basic clerical functions.
Participates in follow-up activities and tracking clients, schedules appointments and performs other related tasks.
Provides case finding and recruitment of at risk persons for comprehensive health services.
Performs other related duties as assigned consistent with the responsibilities of the position and agency operations.
Level Descriptor
This is the specialist level of this job family where employees are assigned responsibility for providing more complex patient care tasks and performs more advanced duties.
Knowledge, Skills, Abilities and Competencies
Candidates must possess sufficient proficiency in the English language to perform the essential functions of the position.
Education and Experience
Education and Experience required at this level consists of two years of experience in providing patient care. Must have current Long-Term Care (CNA) certification and Certified Medication Aide certification with the four advancements (Gastro, Respiratory, Glucose and Insulin).
