Responsibilities

• Deliver occupational therapy interventions within an intermediate care service , supporting patients in their own homes or community settings.
• Complete functional assessments and develop individualized rehabilitation plans.
• Work collaboratively with the multidisciplinary team to ensure safe and timely discharges.
• Promote independence and optimize patient outcomes through evidence-based practice.

Requirements

• HCPC registration as an Occupational Therapist.
• Experience in community rehabilitation or intermediate care .
• Strong understanding of discharge pathways and admission avoidance strategies.
• Excellent communication and teamwork skills.
• A valid driving licence and access to a car (essential for community visits).
