Millennium Home Care, a leading provider of in-home healthcare services, is looking for a dedicated full-time Physical Therapist to join our team. Provides initial and ongoing comprehensive assessments of the client's needs including Out c o m e and Assess m ent Info r m ation Set (OAS I S) assess m ents. Establishes a treat m ent plan in consult a tion with the physician and revises it with the app r oval of the physician as necessary. Assists in d e velop m ent and i m ple m entation of the interdisciplinary client care plan as it pertains to Physical Therapy. Assists the physician in eva l uating patients by a pp l ying diagnostic and p r ognostic muscle, nerve, joint and functional ability tests. Treats clients to relieve pain, to develop or r estore function, and, to m aintain m axi m u m perfor m an c e using physical m eans such as exercise, massage, heat, water, li g ht and electricity. Consul t s and fa m iliarizes s t aff regarding t h e established plan. Educates, supervises, and cou n sels the family and client in the physical thera p y program. Instructs client in care and use of wheel c hairs, braces, crutches, canes and prosthetic and orthotic devices. Provides the equipment required to carry out the plan. Arranges for out-patient service for p r o c edures that cannot be given at ho m e. Re c o rds all evaluation data, treatments and the client ' s response to therapy intervention on a timely basis. Also r ecords quality assurance data. Prepares an assessment of the client's condition, periodic evaluations and s u mmaries. Superv i s es Physical The r apy Assistants and parap r of e ssionals at least once m onthly. Records and reports to t h e physician the client ' s reac t ion to treatment and /or any changes in the client ' s condition. Co mmu nicates with the phys i cian at least every 30 days or whenever changes occur regarding t h e client's status and o b tains addi t ional o r de r s , if necessary. Communicates with other team members ( i nclud i ng ho m e h ealth aides) and, wh e n app r opriat e , instructs them in certain phases of physical therapy that they may use while working with the client. Enters clinical notes within required hours of scheduled visit (24 hrs. for non-Oasis visits / 48 hrs. for Oasis visits) and progress notes and other clinical record forms outlining the services rendered. Participates in case conferenc e s, physical therapy staff meetings, agen c y activities and in-service programs as necessary. Superv i s es the ho m e health aide every two weeks when physical therapy is the only professional discipline involved in the client ' s care. Participates with staff, clients, and physicians in discharge planning activities and completes a Physical T h er a py Discharge Su mm ary. Is knowledgeable of the Agency's organization, functions, and services. Complies with Agency dress code policy. Maintains efficient flow of Agency work by reporting to work at assigned or scheduled times; follows procedures for notification of absence or time off requests. Maintains current TB testing (refer to Health Screening Policy).
