About the role

In this position, you will deliver specialist Occupational Therapy services to children and young people with disabilities. You will assess individual needs, recommend appropriate interventions, and support families in creating safe and functional home environments. Working collaboratively with professionals across social care, health, and education, you will play a key role in delivering effective and person-centred outcomes.

Responsibilities

  • Assess children and young people to identify functional needs and develop effective intervention plans.
  • Recommend and arrange specialist equipment and minor adaptations.
  • Demonstrate the safe and effective use of equipment to parents and carers.
  • Assess the need for major home adaptations and guide families through the adaptation process.
  • Liaise with Grants Officers, surveyors, architects, builders, and other professionals to ensure adaptations meet assessed needs.
  • Complete housing needs reports to support medical priority housing applications.
  • Maintain accurate records, assessments, risk assessments, care plans, and reviews.
  • Attend multidisciplinary meetings and work collaboratively with partner agencies to achieve positive outcomes.
  • About You
  • Degree in Occupational Therapy and HCPC registration
  • Experience working with children with disabilities or complex needs
  • Strong understanding of assistive equipment and housing adaptations
  • Experience in functional and environmental assessments
  • Excellent written and verbal communication skills
  • Ability to manage a caseload independently
  • Experience working within a multidisciplinary team environment
