Learn more at shn.ca Job Description: The First Episode Treatment Team (FITT) is seeking a Child and Youth Intake Worker to bring a blend of specialist and generalist skills to our multidisciplinary team. The successful candidate will focus primarily on providing case management, service navigation and the delivery of Individual Resiliency Training (IRT). Following NAVIGATE and Ontario standards for early psychosis intervention, you will actively navigate the intersection of clinical interventions and case coordination to ensure patient-centered care. Core Responsibilities Assessments & Interventions: Conduct thorough psychosocial assessments and provide responsive interventions for outpatient youth and young adults Integrated Case Management: Balance clinical interventions, psychoeducation and case management. Care Coordination: Maintain an excellent knowledge of community resources to facilitate active liaison, referral, and continuity of care. Team Collaboration: Function as an active, effective member of our multidisciplinary mental health team to optimize client outcomes. Qualifications & Experience Bachelor's Degree in Child & Youth Work, Child and Youth Diploma, or related mental health field education Minimum of 5 years of dedicated experience in child and adolescent mental health, preferably within an outpatient clinic setting and with individuals experiencing early psychosis Previous experience in assessment, crisis intervention, and the planning and development of therapeutic programming in an outpatient child & adolescent mental health clinic Proven excellent communication (English oral and written), and interpersonal skills dealing effectively with clients, families and community partners Excellent knowledge of community resources, participating in liaison and referral Demonstrated ability to function as an effective member of the multidisciplinary team Knowledge of the Mental Health Act and relevant legislation Demonstrated pattern of good attendance Demonstrates an understanding of and commitment to Scarborough Health Network's comprehensive safety programs and practices Demonstrates a commitment to customer service when dealing with patients, colleagues, volunteers and other members of the community and public.
