Foundation Medical Partners, our multi-specialty group, spans 70+ locations across southern New Hampshire and northern Massachusetts, providing coordinated, patient-centered care to thousands each year. The Registered Nurse in our Primary Care Practice provides professional nursing care to patients of all ages in collaboration with physicians, advanced practice providers, and the healthcare team. This role supports both routine and complex patient needs, delivering care in-person, by telephone triage, and through telehealth. Responsibilities include health assessments, patient education, chronic disease management, preventive screenings, medication administration, diagnostic testing, and coordination of care. The RN plays a key role in ensuring patients receive timely, compassionate, and high-quality primary care services.
