Education: Bachelor's degree in Health Administration, Health Sciences, Public Health, Nursing (BSN) or equivalent degree required. Masters preferred.
Licenses/Certifications:
Certified Professional in Healthcare Quality (CPHQ) or Healthcare Quality and Management (CHCQM) preferred
Experience / Knowledge / Skills:
Minimum five (5) years of experience in programmatic population health quality care gap closure required
Five (5) years of supervisory experience preferred
Prior experience in Epic electronic medical record strongly preferred
Self-motivated, high impact communication skills, assertive
Background in healthcare business planning, and targeted outcomes
Working knowledge of managed care, value-based care, population health, inpatient, outpatient, risk adjustment, and annual wellness visits
Working knowledge of the concepts associated with Performance Improvement

PRINCIPAL ACCOUNTABILITIES
Designs and implements the ACO Quality operating model through the ACO Quality and RAF Operations Committee
Responsible for Medicare Shared Savings Program (MSSP) performance and operations. Leverages available MSSP data and analytics insights and partners with MSSP ACO Participating physicians to improve quality and outcomes. Ensures compliance with MSSP program requirements.
Works to develop ACO quality outcomes reporting in partnership with AVP Population Health and the VBC and Population Health Analytics support team.
Ensures that ACO Quality program efforts are aligned to support MHMD, MHMG, and the Primary Care Collaborative.
Responsible for approving and managing the day-to-day ACO Quality and Network Performance operational budget.
Assures that revenue, expenses, contribution margin and FTE's meet or exceed budget.
Prepares and submits budget and related reports.
Forecasts and accurately projects expenses.
Takes corrective action to address negative variances.
Identifies and proposes capital budget items appropriately.
Responsible for leading a high performance team of "system thinkers" who incorporate leadership principles and vision in performing the functions of their roles.

Job Summary
The Director of Population Health Performance is responsible and accountable for the Enterprise implementation of ACO Quality, Medicare Shared Savings Program (MSSP), and MHMD Network Performance. The components/roles of the ACO Quality Team consist of ACO Quality Coordinators, synchronous and asynchronous patient engagement, and MHMD/MHMG/High Performing Physician Network support for ACO Care Gap closure. The components/roles of the Network Performance Team consists of Practice Advisors collaborating with independent practice members of MHMD.
The Director is responsible for organizing all aspects of data collection, report compilation/dissemination, and coordinated outreach efforts for ACO Quality, MSSP, and Network Performance efforts.
