Ensure compliance with Medicare hospice regulations and ACHC standards
Review and validate certifications and recertifications, face-to-face encounters, hospice eligibility, and documentation of decline
Review plans of care and visit documentation for accuracy and completeness
Identify documentation gaps and support timely corrections
Track trends in live discharges, revocations, length of stay, and documentation inconsistencies
Partner with clinical leadership to ensure follow-through and accountability
Maintain ongoing survey readiness through consistent chart review
Provide clear, constructive feedback to clinicians to strengthen documentation
What You Bring
Active RN license (California)
Hospice experience required
Strong understanding of hospice eligibility, recertification requirements, and documentation supporting terminal prognosis and decline
Knowledge of Medicare hospice compliance standards
Experience in QA, auditing, or clinical review preferred
Strong attention to detail with a patient-centered mindset
Ability to communicate clearly and work collaboratively with clinicians

Position: Full-Time | On-Site
Compensation: $55 - $65/hour DOE

We are seeking a Hospice QA Lead based in our San Francisco office, focused on chart review, documentation accuracy, and regulatory compliance. In this role, you will help ensure that each patient's clinical needs, eligibility, and care are clearly and accurately reflected in the chart, while keeping the agency aligned with ACHC standards and Medicare requirements.
