Participates on interdisciplinary teams that evaluate clinical practice or health services relating to patient/resident care.
Participates in QI studies: involved in task force or committee. Implements suggestions for improving outcomes per unit study results. Independently identifies problematic areas and recommends problem solving. Shares findings of studies, audits with others. Seeks improvement independently.
Collaborates and communicates with the patient/resident, family, and other health care providers. Utilizes peer review and observation. Monitors documentation for appropriate referrals:
Formulates the plan of care and involves patient/residents in decisions related to care and the delivery services.
Makes referrals, including provisions for continuity of care, as needed.
Is a primary clinical expert and resource for patients with complex wounds, or ostomy care issues, along with incontinence issues.
Manages and prioritizes orders to ensure the service is provided to patients with the greatest need first to maximize quality of care, and minimize length of stay.
Ensures collaboration between staff, physicians, other disciplines and departments involved in the care of the patient. Keeps lines of communication open.
Provides staff with support, guidance, and educations regarding wound/ostomy/incontinence care and alerts the Nurse Manager as needed for any remediation issues.
Makes fiscally responsible decisions regarding use of time, supplies, and equipment, and considers the organizational fiscal issues when making decisions regarding treatment pl

The purpose of this position is to provide expert level nursing care, clinical assessment, and recommendations for the treatment of patients with complex wounds, ostomy, or incontinence issues. The acute wound care nurse works in partnership with other registered nurses, ancillary staff, and physicians to provide education and recommendations for an individualized plan of care for patients who require their specialized training and expertise.
