POSITION SUMMARY: A Population Health Post - Acute Care Navigator works with primary care physicians, specialists, outcomes management, and other services to maximize health services and coordination for patients. This position requires a high frequency of contact with patients admitted to the hospital or in a skilled nursing facility. A Population Health Post-Acute Care Navigator will ensure the coordination and communication of a patient's treatment plan and general status to all care givers during all transitions of care. This position requires knowledge of the health care system and expertise to identify and implement improvement processes and the ability to direct and implement care coordination plans both in both in-patient and out-patient settings. EDUCATION: Graduate form an accredited School of Nursing or Physical Therapy with a minimum of Bachelors Degree in respective field
