About the role

We are currently seeking a dedicated and experienced Team Leader / Rehabilitation Support Worker to support an 18-year-old male client living in the CR3 area. This is a highly rewarding opportunity to work within a dynamic multidisciplinary team (MDT) , supporting a young individual with complex needs through a structured rehabilitation pathway. The role focuses on neurorehabilitation and spinal injury support , bridging the transition from hospital to home. Support rehabilitation programmes and therapy goals
Assist with manual handling and mobility support

Provide emotional and psychological support to the client
Help prevent complications and promote long-term wellbeing
Person Specification
Essential Skills & Experience
Experience supporting individuals with spinal cord injuries and complex needs
Strong manual handling skills
Experience in medication administration and monitoring (hospital/ICU background desirable)
Understanding of rehabilitation and recovery pathways
Excellent communication and interpersonal skills
Good IT and documentation skills
Ability to work independently within a client's home
Willingness to learn, adapt, and take feedback
Desirable
Experience supporting clients in community-based settings
