The Care Continuum Partner - Medical Assistant functions in a regional capacity. The Care Continuum Partner - Medical Assistant is a role that supports patients and practices collectively. Responsible for meeting patients where they are within our physical/virtual walls and responsible for advanced level partnerships to support patients outside of the practice walls. Job Duties In collaboration with clinical care team, participates/supports pre-visit planning to ensure smooth operation and patient flow within the practice. Conducts associated workflows for care gap closures, referral and medication authorizations. Supports care plan compliance by conducting patient outreach to follow up on patient care and appointment needs. Communicates with the patient in traditional and innovative platforms including telephone, patient portal and other virtual platform solutions where applicable. Facilitates virtual rooming and virtual appointment concierge calls for telehealth appointments. Provides the patient with clear instructions on how to prepare for tests/procedures and provide education on what to expect per established guidelines. Supports the intake for annual well visits. Manages visit prior authorizations and referral appointments. Supports empanelment for assigned practices. Serves as expert and liaison to patient/family on the patient portal and medical record technology. Functions as a virtual care concierge partner offering assistance and guidance as appropriate. Minimum Qualifications Technical School Diploma as a Medical Assistant or Specialized Diploma as a Medical Assistant 2 years experience interacting with patients and performing identified core functions of a MA. and 3 years experience working within a complex health system environment or related field. High degree of accuracy and attention to detail. Enhances skills through continuing education. Ability to handle numerous tasks at the same time. Superior customer service skills. Working knowledge of basic computer functionality. Effective and professional written and verbal communication. Ability to maintain strict confidentiality. Ability to adapt to changing priorities Critical thinking skills and solution focused approach. Preferred Qualifications Associate's Degree Medical Assisting Knowledge and experience with electronic medical records and scheduling software. Knowledge of medical terminology. CCMA - Certified Clinical Medical Assistant - NHA - State of Pennsylvania Physical Demands Lift and carry 25 lbs. frequent sitting/standing, frequent keyboard use, *patient care providers may be required to perform activities specific to their role including kneeling, bending, squatting and performing CPR.
