About the role
Under the direct supervision of the Nursing Manager, the Hospice Nurse Case Manager is responsible for coordinating and delivering compassionate end-of-life care to patients and their families. Support patients and families in making informed decisions about care and treatment options. Documentation & Compliance: Maintain accurate, compliant, and timely records of patient care, interventions, and progress while adhering to hospice regulations. Home & Facility Visits: Travel to patients' homes, nursing facilities, and hospitals to provide direct care. Professional Development: Continuously seek opportunities for learning and skill enhancement in hospice and palliative care and care management. Actively participates in safety initiatives and risk mitigating measures where appropriate and completes all position and unit safety related competencies and requirements on a timely basis.
