About the role

This position is located in the Community Care Service Line at the Tomah VA Medical Center. The primary purpose of the position is to serve as a Community Care Coordinator. This position works collaboratively with the patient care teams as well as a variety of specialty health care providers. Examples of specialized experience would typically include, but are not limited to: Generating authorizations for pre-authorized medical care, obtains requested reports, x-rays, and labs, coordinate transportation for Veterans as needed, updating and verifying demographic information, verifying insurance information, data entry, managing consultations, and notifying the patients of his/her schedule appointments.
