Title: Case Manager Job Duties: The Case Manager is responsible for assisting in planning, developing, organizing, implementing, and evaluating the Activity Program and Special Treatment Program (STP) in accordance with current existing federal, state, and local standards, as well as established policies and procedures. The Case Manager is responsible for case management in order to ensure that residents are given ample and regular opportunities to learn and practice skills for improving the level of functioning, independence, awareness, self-care, and self-esteem.
