MSW degree and registered with the Ontario College of Social Workers and Social Service Workers
Experience in a hospital setting
Excellent crisis intervention skills
Strong focus on psychosocial assessments and discharge planning
Excellent assessment skills and familiarity with community services
Strong ability and interest in linking patients to community resources
Must be flexible and a strong team player
Demonstrates excellent interpersonal and communication skills (verbal & written) and good judgement when relating to patients, families, co-workers and interdisciplinary teams
