Job Title: Social Worker - Learning Disabilities Location: Kingston Upon Thames Pay Rate: £32 per hour Job Description: Liquid Personnel is currently recruiting a Social Worker to join its client's Learning Disabilities Team based in Kingston Upon Thames. This role focuses on supporting young people aged 17 to 25 as they transition from Children's Services to Adult Social Care. You will empower individuals to navigate this critical phase with confidence, independence and dignity-ensuring their needs are met through person-centred planning and multi-agency collaboration. What will your responsibilities be? Complete Care Act assessments and develop outcome-focused support plans. Undertake prevention and enablement work, including direct interventions and reviews. Carry out Mental Capacity Act assessments, Best Interests decisions, and DoLS applications. Respond to safeguarding concerns and lead Section 42 enquiries. Implement safeguarding plans and apply Making Safeguarding Personal principles. Apply a strengths-based approach to support young people and their families. Participate in Pathway Plans, EHCP reviews, and IRO meetings. Explore funding options including Continuing Healthcare and Section 117 aftercare. Develop effective partnerships and joint working arrangements to ensure smooth transitions. Manage a diverse caseload and participate in weekly duty rotations. Maintain accurate and timely records, including assessments, case notes, and funding applications.
