STATUS: Per Diem
LOCATION: Golisano Rehabilitation - 1555 Long Pond Rd. Rochester, NY
DEPARTMENT: Rehabilitation/Speech Pathology
SCHEDULE: Day shift, 8-hr. shifts, 8:30 am - 4:30 pm (per diem as assigned)
occasional weekends & holidays
Responsibilities
Documentation. Maintain a complete documentation of patient evaluations, treatment, progress, and discharge status; send required information to appropriate professionals
Collaboration. Actively participate in rehabilitation team efforts to meet patient needs, including counseling and teaching patient and family, discussing
changes in treatment programs with MDs, coordinating healthcare plans with other involved professionals, and assisting in discharge planning as appropriate
REQUIRED QUALIFICATIONS:
Master's degree in Speech-Language Pathology or Communication Disorders (may be MA, MS, Med)
Required Licensure/Certification Skills : New York State Licensure
Minimum of nine (9) months of supervised work experience (completion of CFY), or participation in supervised clinical fellowship, for a minimum of 36 weeks under a NYS licensed Speech Pathologist, followed by the NYS licensure examination and achievement of NYS license
Compassionate, warm, and patient-focused
Excellent organization and planning skills
Requirements
One (1) year of experience with the frail elderly population is preferred

M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.
