Essential Functions
Care Coordination and Advocacy:
Serves as a leader and partner on the interprofessional healthcare team to enhance care coordination across settings and among caregivers.
Provides consultation and collaboration to design, coordinate, and evaluate plans of care.
Mentors and leads staff to be patient advocates to actively involve the patient/family in the care plan development.
Care Environment Management:
Delegates and uses nursing team resources and services as a partner in the interprofessional healthcare team.
Evaluates clinical and cost outcomes and identifies needed changes in systems and processes supporting the care environment to improve safety, effectiveness, timeliness, efficiency, quality, and patient centered care.
Uses information systems and technology to improve outcomes.
Education:
Systematically plans and evaluates overall nursing staff development in response to patient and healthcare trends, regulatory requirements, and organizational strategic priorities.
Uses appropriate teaching/learning principles and strategies and current information, standards, and technologies to facilitate learning of patient/family and healthcare team.
Conducts and communicates comprehensive outcome evaluation of educational activities on learner and organization.
Outcomes Management:
Assumes accountability for outcomes for specific populations within a unit or setting.
Designs/redesigns client patient care based on analysis of outcomes and evidence-based knowledge.
Raises questions to challenge existing practices to promote evidence-based practice (EBP).
Synthesizes literature and research findings to design, implement, and evaluate interventions for selected problems or populations.
Fosters staff's knowledge and skill in the research/EBP process and the application of EBP and research.
Professional Development:
Effects change through advocacy for the profession, interprofessional healthcare team and patient.
Promotes professional nursing role competence.

$72.290 - $93.280 - $104.470
