In the Health Access Nurturing Development Services (HANDS) program, responsible for reviewing clinic records and interviewing / conducting Parent Visit Assessment/Survey with parents (prenatal and/or immediately after the birth) to determine appropriate level of support from the community. Families facing challenges will be offered visits by a Family Support Worker, if the family opts out of participation they will be referred to appropriate community resources based on family needs MINIMUM REQUIREMENTS: MINIMUM REQUIREMENTS ARE COMPREHENSIVE STATEMENTS OF THE MINIMUM BACKGROUND AS TO EDUCATION, EXPERIENCE, AND OTHER QUALIFICATIONS WHICH WILL BE REQUIRED IN ALL CASES AS EVIDENCE OF AN APPOINTEE'S ABILITY TO PERFORM THE WORK PROPERLY. EDUCATION: Bachelor's Degree in Social Work, Nursing, Early Childhood Education or a Bachelor's degree in a Social or Behavioral Science related field may also be considered with one (1) year experience performing case management services.

  • A Master's degree in Human Services may substitute for the one (1) year of experience.
  • Associate degree in Nursing with licensure in Kentucky or a compact state and two (2) years of public health experience will substitute for education.

  • POST EMPLOYMENT REQUIREMENTS: EMPLOYEES IN THIS CLASS WILL BE REQUIRED TO OBTAIN CREDENTIALS OUTLINED BELOW WITHIN A REASONABLE TIME, OR IF EMPLOYEES POSSESS SUCH CREDENTIALS AT THE TIME OF HIRE THEY WILL BE REQUIRED TO MAINTAIN THOSE CREDENTIALS SO LONG AS THEY ARE EMPLOYED IN THIS CAPACITY.
