About the role
Works as part of a care team to improve the execution of quality requirements, increase throughput, improve efficiency, and enhance the overall patient experience. Provides care to designated patients and care coordination across the continuum. Performs administrative functions in the practice. Job Duties Assists provider by collecting patient history and reason for visit. Completes observation, physical exam, telephone triage, and analysis of diagnostic data. Ensures that all ordered laboratory and diagnostic tests are tracked and logged and that the provider is presented with all test results. Provides education to patients, ensures education materials are current and appropriate, and evaluates the effectiveness of patient education. Assists patients with inquiries for prescription refills, appointment scheduling, and referral authorization. Assists providers during procedures and examinations and performs instrument and room clean-up. Coordinates internal and external appointments for new and established patients based on patients' medical needs, preferences, and the practice schedule. Intervenes to prevent, minimize, or correct any actual or potential patient risks based upon observation of physiological, behavioral, and environmental factors. Participates in telephone triage by obtaining clinically appropriate data and symptoms; advises and instructs patients according to established protocols and/or orders from the provider. Minimum Qualifications Technical School Diploma LPN Program Ability to establish and maintain control of violent patients or dangerous situations. Basic computer skills, critical thinking, and ability to concentrate. Knowledge and competence that reflects current nursing practice.
