Pay range: $44.01 - $66.02
Schedule: 7:30-6pm Monday, Tuesday, Wednesday, Friday
This position is eligible for a $5,000 sign-on bonus and paid relocation assistance (in-state or out-of-state, amount based on location). Bonuses are available to external candidates only with at least one year of relevant experience who have not been employed by St. Charles Health System within the past year. All bonuses are subject to applicable tax withholdings.

DEPARTMENTAL SUMMARY: The Rehabilitation Department provides therapy and nursing services to patients, families and patient's caregivers including: Rehabilitation Nursing Care, Physical Therapy, Occupational Therapy and Speech Language Pathology. Our department operates as a system department and provides services to Acute Care, Inpatient Rehabilitation Facility, Home Health and Outpatient settings across four campuses. Our Rehabilitation departments offer a variety of specialty services across the various services delivery areas.
POSITION OVERVIEW: The Occupational Therapist evaluates, plans, and provides treatment and rehabilitation as prescribed by a referring physician. Works with family and team members regarding patient's abilities and goals.
This position does not directly manage any other caregivers.
ESSENTIAL FUNCTIONS AND DUTIES: RESPONSIBILITIES:
Adheres to Oregon State Occupational Therapy Licensing Board.
Provides occupational therapy evaluation and treatment to achieve desired outcomes.
Provides staff consultation and training.
Provides community resource knowledge and referral services.
Provides occupational therapy services to other hospital service areas.
May be required to drive Rehabilitation van and meet all related SCHS vehicle policy requirements.
Functions as a member of the Interdisciplinary Care Team to assure patient care is accomplished effectively and efficiently in a cost effective manner.
Participates in clinical care conferencing.
Gathers data and accurately documents information in a timely manner demonstrating compliance with departmental and professional regulations.
Participates with team members in facilitating patient's and family's learning throughout the continuum of care. Reinforces patient's continued health care through teaching and/or referral to community agency follow-up.

EDUCATION:
Required : Bachelors, Masters, or Doctorate degree in Occupational Therapy from a college or university accredited by the American Occupational Therapy Association

LICENSURE/CERTIFICATION/REGISTRATION:
Required : Must have necessary requisites for licensure as an Occupational Therapist by the Oregon State Board of Health. Valid Oregon driver's license and ability to meet St. Charles Health System driving requirements. Ability to travel to business functions/trainings/meetings and all St. Charles Health System worksites. Current AHA Basic Life Support for Healthcare Provider certification.
