About the role

If so, we're looking for Carer Hub Navigators -who will be the welcoming face and guiding hand for carers in our new Carer Hubs. This is an exciting new role designed to integrate community development and community engagement approaches to raise awareness and enhance accessibility to Carer Gateway services through in our Carer Gateway Hub. The Carer Services Navigator role is based in the Toowoomba region, providing consistent presence and reliable support to carers however adopts a 'no wrong door' approach. Working in collaboration with a team, you will actively build strong, sustainable relationships with community organisations, health services, schools, aged care facilities and local government agencies to further strengthen and embed carer support locally. The role has four core functions: To create seamless connections between Carers and vital community resources, cultivating meaningful relationships that support wellbeing and independence. Supporting Carers to thrive, focusing on holistic well-being, going beyond basic needs to help carers achieve their full potential. The service is Carer focused, 'how can we help' and a 'no wrong door approach'. Oversee the Gladstone Carer Hub, facilitating social interaction and promoting wellbeing, connection and a sense of belonging. To be successful in this role, you will need: Experience working with family carers in mental health, alcohol and other drugs, community support services Certificate IV or above in a support discipline and/or Equivalent Community sector experience Current valid Driver's Licence Strong technology skills Satisfactory National Police Records Check (within 12 months) WWCC / QLD Blue Card NDIS Workers Screening check / QLD Yellow Card Appropriate IT Skills Right to Work within Australia NDIS Worker Orientation Module Certificate
