Job Description Summary: Works with individuals, groups, or communities to facilitate positive and healthy behaviors. Interacts with adolescents who have congenital heart disease using a theory-based lifestyle intervention to increase physical activity. Job Description: Essential Functions: Conducts assessments to identify behaviors and develop intervention plans. Monitors and evaluates the effectiveness of interventions and modifies plans as needed. Collaborates with other professionals, such as psychologists and social workers, to provide comprehensive care. Provides training and support to caregivers and other professionals to ensure consistency in intervention strategies. Maintains accurate and up-to-date records of client progress and communicates with relevant parties as needed. Maintains accurate and timely documentation of all interventions and progress. Education Requirement: Bachelor's degree in a relevant area, including exercise physiology, nursing, physical therapy, psychology, community wellness, social work, or another health-related field, required.
