Job Overview
The Case Manager provides collaborative, court-involved community-based care by working closely with case managers, acute care teams, psychiatrists, hospitals, and the court system to support client wellness, long-term stability, and prevent rehospitalization. Engages clients in regular face-to-face and weekly sessions aligned with court-ordered treatment plans, coordinating mental health services, medications, family/support systems, and safety while maintaining timely, accurate court documentation. Communicates professionally and effectively with supervisors, referral sources, and external partners, demonstrating responsiveness, accountability, and commitment to agency standards and continuous improvement.
Program Info
Involuntary Outpatient Commitment (IOC) serves individuals diagnosed with serious, persistent mental illness who, due to his/her mental illness are dangerous to him/herself, others or property in the reasonably foreseeable future. These individuals have also demonstrated a history of positive treatment response in an outpatient setting. IOC is an opportunity to effect change before an individual reaches a crisis point which results in an inpatient hospitalization. The individual must have some level of willingness and capacity to participate in the IOC program and plays an active role in the development of his/her treatment plan.
