Physical Therapist

Responsibilities

• Provide rehabilitative care and skilled physical therapy services to patients in their homes
• Develop and implement individualized treatment plans
• Collaborate with physicians, nurses, occupational therapists, speech therapists, and caregivers
• Educate patients and families to promote independence and improve quality of life
• Complete timely clinical documentation using electronic medical records

Requirements

• Active Physical Therapist license in New Jersey
• Graduation from an accredited Physical Therapy program
• Home health experience preferred but not required
• Strong clinical assessment and communication skills

• Additional Information:
• Base Pay, depending upon qualifications and subject to negotiation:
• $60.00 - $72.00 per point ($93,600 - $112,320 annually)
• **$10,000 Sign on Bonus!** for full time PT positions.
