Master's Degree in Nursing as Advanced Practice Nurse OR

Job Summary
The advanced practice provider (APP) at the Ponce de Leon Center provides culturally competent HIV and general primary care as the primary care provider for continuity patient panel with an emphasis on developing trusting partnerships with patients and ensuring continuity of care under the supervision/guidance of attending physician and in collaboration with patients' care team. The APP performs routine follow up visits, walk in/urgent care visits, and comprehensive new patient/re-enrollment visits including antiretroviral initiation/monitoring in patients with advanced HIV disease with significant psychosocial barriers to care. APPs provide consultation to staff nurses, medical staff, and interdisciplinary colleagues, demonstrating clinical expertise, critical thinking, and problem-solving skills to influence the plan of care for patients/families, optimize clinical outcomes, and affect change in the system.
