Professional knowledge and/or experience family health and/or chronic disease prevention and management activities, including care coordination to manage clinical care and address health-related social needs or social determinants of health.
Experience in oversight, implementation, and evaluation of public health and preventive health programs, as well as promoting public health messages.
Experience coordinating training activities and delivering technical assistance.
Experience contributing to policy development.
Demonstrated experience presenting information to different audiences, including community partners, healthcare providers, leadership, and/or public audiences.
Experience using spreadsheets, databases, and electronic document management systems to support program administration, reporting, or project management.
Conditions of Employment
Grant funded position. Compliance with funding requirements such as time and effort reporting, grant deliverables, and contract deliverables, is required.
Valid and unrestricted authorization to work in the U.S. is required. Visa sponsorship is not available for this position.
The selected candidate must reside within a reasonable commuting distance of the official work location specified in the job posting and must also be located in or willing to relocate to one of the following states: New York, New Jersey, Connecticut, Vermont, or Massachusetts, prior to hire.
Travel, up to 25%, will be required.
This position is eligible for a hybrid work schedule, with a combination of onsite and remote work.
