The Community Health Nurse provides comprehensive community and clinic health services directed towards the prevention of disease, the promotion of health, and the clients self care using professional judgment in comprehensive assessment, screening, nursing diagnosis, care planning, counseling, intervention, case management, health information, education, referral and evaluation. Provide regular screening, comprehensive assessment and interventions for on-going health issues (e.g. depression, diabetes, asthma, wound and pain management, and other chronic diseases). Assures completion of referrals and coordinates care with other healthcare providers. Collaborates with the healthcare team to adapt the plan of care. Specifically knows the African American community, historical and current barriers to health services and has knowledge of the traditions, values and customs. Team Collaboration Assesses clients needs for in-person and telephone response for urgent and non-urgent medical information. Provides health education and promoting self management. Responds to emergency events as needed. Administer medical treatments and medications. Assess, diagnose and prescribe treatment according to standing orders and established protocols. Acts as a resource for team needs. Maintains clinical skills as dictated by the client population and the clinical practice (adult, pediatric, OB, internal medicine). Reviews cases with the providers and team as needed. Seeks expert advice on issues for clients and provides input to providers and nurses on client response. Completes competencies required for the position. Completes documentation and charting as required by policy.
