Medical Social Worker - Social Services - Day Shift Including Every 4th Weekend Both Saturday and Sunday

The role of the Medical Social Worker is to work with the interdisciplinary team to ensure a safe discharge of all adult patients. Duties include but are not limited to: - Identifies patients with barriers to a safe discharge plan. - Completes psychosocial assessments on all patients. - Documents all clinical assessments, notes, and referrals in Epic. - Completes screens and coordinates all new skilled nursing facility and rehab facility referrals. - Assesses all cases involving child abuse/neglect, domestic violence, elderly abuse, and sexual assault. - Educates patient/family about post-acute care options and the role of choice. - Educates patient/family about managed care and payor issues This is a full-time day shift position 9:00am-5:00pm Monday-Friday including every 4th weekend, both Saturday and Sunday, located at NewYork-Presbyterian Brooklyn Methodist Hospital. Preferred Criteria At least one year of experience in an acute care setting Discharge planning experience Experience with Epic and Allscripts
